Our Outreach Support Services provide person-centred, goal-focussed support for individuals referred to us through partner agencies and healthcare professionals. Working collaboratively with each person, we develop tailored support plans that focus on practical goals, improving wellbeing, and increasing confidence and independence within the community.

Support is time-limited and reviewed regularly, with the duration determined by individual needs and agreed outcomes. We encourage clients to actively engage with the support process, attend agreed appointments, and work collaboratively towards achievable goals. Throughout our involvement, we aim to strengthen existing support networks, connect people with appropriate community and professional services, and help establish sustainable support structures that promote long-term wellbeing and positive change.

Frequently Asked Questions

What is expected of me during Outreach support?

We encourage you to take an active role in your support. This includes attending agreed appointments, engaging with the support process, and working collaboratively with your Outreach Worker to achieve your agreed goals.

Can an Outreach Worker attend appointments with me?

Yes. If appropriate, an Outreach Worker can accompany you to appointments to provide reassurance and support. However, they cannot attend appointments on your behalf.

Can the Outreach Service provide personal care?

No. Our Outreach Service does not provide personal care, such as assistance with washing, dressing, toileting, or other personal care needs.

Can Outreach Workers administer my medication?

No. Outreach Workers are not able to administer, handle or manage medication. If you need support with medication, we can help you identify the appropriate service to meet your needs.

Can an Outreach Worker collect shopping, prescriptions, or other items for me?

No. Outreach Workers cannot collect goods, shopping, prescriptions, or other items on behalf of individuals. Instead, we can support you to access services or develop the confidence and skills to do these tasks independently where appropriate.

Will you help me access other services?

Yes. A key part of our role is helping people connect with appropriate community groups, health services, and other professional organisations. We aim to strengthen your existing support network and help establish sustainable support that continues after our involvement ends.

What kind of support can the Outreach Service provide?

Our Outreach Workers can help you to:

  • Develop confidence and independent living skills
  • Access community activities and local services
  • Build and strengthen support networks
  • Accompany you to appointments and offer morale support
  • Set realistic goals and work towards achieving them
  • Connect with other organisations that can provide ongoing support where needed

What happens when my Outreach support comes to an end?

As your support progresses, we will work with you to ensure you have the right support networks and services in place. Our aim is to help you build the confidence, skills, and connections needed to maintain your wellbeing and continue making positive progress after your Outreach support has finished.
